I agree to assume full responsibility for any risks and release the instructor and associated venues from liability for injuries sustained during class participation. Health & Medical Conditions: I confirm that my child is in good health and physically able to participate in dance classes. I will inform the instructor of any relevant medical conditions or physical limitations. Drop-off & Pick-up: I understand that I am responsible for timely drop-off and pick-up of my child and that the instructor is not responsible for my child outside of scheduled class times.
